Who We Are: Chronic SENIOR Care is a medical practice that is paired with CCMG. Chronic Care Management Group is part of a separate MediCare funded program for individuals with ongoing chronic health issues. Chronic SENIOR Care is a physician based organization that is 100% committed to managing your chronic medical conditions. We work in partnership with your primary care provider to manage your chronic conditions in-between your primary care provider visits. You maintain the exact same relationship with your primary care provider as you have in the past, however, in addition you have a separate team that provides 24/7 coverage and has a professional relationship with your provider.

If you do not have a primary care provider, Chronic SENIOR Care and CCMG will provide you with that service. Their is no additional cost to you when you enroll in MediCare CCM program. Our team of physician led clinicians will initially interview you, provide a complete physical exam, and secure appropriate laboratory work as needed. Each month our team discusses, plans, and if indicated, adjusts the management of your chronic health conditions. If necessary, we will contact you via phone, and if the situation warrants we will request a home visit. We are in constant contact with your primary care provider and always invite any input that he or she has towards improving your health status.

Chronic SENIOR Care also works directly with state and federal social service entities to provide the services you need and deserve.
