Chronic Care Management is the care coordination that is outside of the regular office visit for patients with multiple (2 or more) chronic conditions expected to last 12 months, that place you the patient at significant risk of death, acute exacerbation, decompensation or physical decline. CCM care is totally separate from your primary care provider's care and works in partnership with your provider. CCM provides help between your previous provider's office visit and your upcoming visit.
